Health Assistant
About the Role
The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients at Old Town Clinic. The Care Team is responsible for proactively managing the healthcare needs of patients, including behavioral and medical health needs. The HA supports the Care Team Managers (CTM) and providers on each primary care team.
Old Town Clinic provides treatment for acute and chronic health concerns, primary care, mental health support, care for substance use disorders, and an on-site pharmacy. The clinic is adjoined to the Old Town Recovery Center and the Richard Harris Building.
Responsibilities
- Receive, document, and direct patient telephone calls in a complete and timely manner according to clinic protocols.
- Prepare prescription refills under provider orders.
- Electronically file and index clinical information into the patient’s chart in EMR.
- Obtain, process, and document all necessary information and paperwork to assist the Care Team Managers (CTM) and providers in delivering health services.
- Provide follow-up contact in person or by phone to patients identified by CTM or other care team members for support and management of health-related issues.
- Participate in outreach activities to engage patients in services, including those due for routine preventive care, chronic disease management, or who have missed appointments.
- Provide educational information and materials to patients regarding available community services and how to access them.
- Coordinate services with all members of the care team and the client’s support network.
- Maintain client records as required by program standards and funding requirements.
- Participate in training opportunities to advance skills and knowledge in primary care, addictions, and mental health.
- Work within the scope of training and experience, seeking consultation when appropriate.
- Attend all mandatory CCC trainings in a timely manner.
- Adhere to all state and federal privacy and security regulations, as well as CCC policies regarding confidentiality, privacy, and security.
- Immediately report any breach of protected health information or personal identification information.
- Perform other duties as assigned or delegated by the Care Team Manager.
Requirements
- High school diploma or GED required; Bachelor’s Degree or higher in health or social services preferred.
- Experience working with homeless, low-income, or other vulnerable populations in a supportive role or clinical setting.
- Spanish language fluency preferred.
- Competencies in effective oral and written communication, understanding healthcare terminology, and providing outreach and supportive services.
- Ability to adhere to Central City Concern’s drug-free workplace policy.
- Current CPR certification before start date.
- Must pass a pre-employment drug screen, TB test, and background check, including clearance by the DHS background check unit.
- Ability to bend, stoop, kneel, squat, twist, reach, pull, lift heavy objects, and climb stairs, with reasonable or no accommodation.
- Ability to interact effectively with co-workers and clients from diverse backgrounds and treat individuals with respect and dignity.

Schedule
Monday - Friday, 8:00am - 5:00pm (Saturday & Sunday off).
Pay
$24.30/hr - $33.25/hr. Final compensation will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates: Offer maximum is $28.20/hr per the Union contract.
This role is a Union Represented position through the American Federation of State, County, and Municipal Employees (AFSCME).
