Responsibilities

• Provides direct face-to-face community-based services to assist clients in obtaining and maintaining housing.
• Conducts screenings, intakes, and assessments with clients to evaluate presenting problems and determine needs, developing appropriate treatment plan goals.
• Provides outreach to engage clients in Permanent Supportive Housing (PSH) services and orients potential clients to the H2O program.
• Provides transportation to clients as appropriate and medically necessary, using personal vehicle or Terros Health provided vehicle when available.
• Ensures ongoing collaboration and communication of clinical information with involved parties (e.g., family, behavioral and medical providers, community resources).
• Coordinates care with clients’ family, behavioral and general medical providers, community resources, and other supports.
• Assists clients with connection to physical health services, including primary care and medical appointment follow-up.
• Provides excellent customer service to clients, internal and external provider agencies, state entities, and other stakeholders (e.g., probation officers, advocates, family members, residential staff, hospital staff, vocational rehabilitation).

Requirements

• High School Diploma/GED with a minimum of 1.5 years of behavioral health experience; or
• Associate’s degree in the field of medical or behavioral health; or
• A bachelor’s degree (not in a related field of behavioral health) and 1 year of behavioral health experience; or
• A bachelor’s degree or higher in the field of behavioral health.

SMI Experience Strongly Preferred

Schedule

• Monday – Friday
• Two shifts available: 7:30am – 4:30pm or 8:30am – 5:30pm
• No on-call or weekends required
• Up to 80% in the field, 20% in the office (Mileage Reimbursement)

Pay

• $17.87–$21.37 per hour (depending on years of behavioral health experience and education level)
• $3/hour Differential Pay
• Additional language differential pay available for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili, or ASL: $1/hour (approximately $2,080 annually)
