Head Fall Cheer Coach

Applicant must possess (or be able to obtain) a currently-valid Pupil Activity Permit issued by the Ohio Department of Education (ODE) or Ohio State Board of Education (SBOE). Please scan and attach the Permit, or provide your State ID#.
