HCHB Specialist, RN

Responsibilities

Lead, inspire, and motivate clinical staff to provide outstanding patient care
Conduct client assessments and supervisory visits throughout Exton and surrounding areas
Develop individualized care plans and oversee implementation of services
Supervise and support RNs, PTs, OTs, SLPs, and other field clinicians
Facilitate new hire orientation and onboarding
Conduct competency evaluations for both new and tenured staff
Review and monitor clinical documentation within Home Care Home Base
Ensure compliance with company policies, regulatory standards, and quality initiatives

Qualifications

Active Registered Nurse (RN) or Physical Therapist (PT) license required
Minimum of 2 years of clinical experience required
Home Health experience required
Prior supervisory or leadership experience preferred
Strong clinical, communication, and organizational skills
Ability to build strong relationships with clients, families, and interdisciplinary teams
