About the Role

3HC Home Health and Hospice is seeking a Home Health Nurse, RN to provide skilled nursing care to patients requiring intermittent professional nursing service. This position offers the opportunity to provide meaningful one-on-one care and develop relationships with both patients and team members in a supportive, family-like work culture.

Coverage Area & Schedule

  • Coverage: Kinston/Goldsboro, North Carolina
  • Schedule: Monday–Friday

Essential Job Functions

  • Provide skilled nursing care as outlined in the physician's plan of treatment, according to 3HC's policies and procedures and as allowed by the Practice Act of the N.C. State Board of Nursing
  • Observe and document signs, symptoms, and reactions to treatments, including drugs and changes in the patient's physical or emotional condition and outcomes of intervention
  • Ensure ongoing evaluation of patient/family needs and response to care
  • Refer to other services as needed
  • Initiate appropriate preventative and rehabilitative nursing procedures for the patient's care and safety
  • Educate, supervise, consult, and counsel the patient/family regarding nursing care needs, disease process, self-care techniques, and issues of ethical concern
  • Assure the development and implementation of the patient individualized plan of care and update as needed
  • Conduct a complete assessment of the patient to identify appropriate care needs, including medical history (including OASIS when needed), physical assessment, support system evaluation, health teaching needs, and discharge planning
  • Document services rendered electronically and download daily, including time and travel documentation
  • Respond within 48 hours to requests for documentation correction/completion
  • Ensure ongoing physician involvement in patient care, including reporting reactions, changes in condition, and consulting on alterations to the plan of treatment
  • Complete OASIS at admission, resumption of care, recertification, and other specified points
  • Provide regular supervision to Licensed Practical Nurses and paraprofessionals providing services to patients according to regulatory and CMS/Hospice guidelines
  • Supervise LPN on every patient at least every 30 days
  • Provide certified nursing assistants with a telephonic plan of care, supervise and evaluate care given at least every two weeks
  • Collaborate with supervisor to meet assigned productivity of 100%
  • Rotate weekday, weekend, and holiday "on-call" and "back-up-call"
  • Maintain regular communication with supervisor and communicate patient problems on a timely basis following chain of command
  • Complete certifications and re-certifications within designated timeframes
  • Keep up-to-date on reimbursement criteria and documentation requirements, including preauthorization and re-authorization for insurance
  • Travel to other offices to meet patient needs as requested by the Director of Clinical Practice
  • Stays abreast of overall patient care and improves nursing care skills through participation in staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services
  • Attend and participate in interdisciplinary team conferences at least 75% of the time
  • Attend agency mandatory in-services and satisfy CEU requirements of the State Board of Nursing
  • Demonstrate interest in personal and professional growth through continuing education, workshops, seminars, webcasts, individual research, and reading
  • Be cost effective in the use of agency resources, monitoring waste, and proper and safe use of supplies and equipment
  • Perform other appropriate duties as assigned by supervisor to promote successful operations and future growth of 3HC

Role-Specific Responsibilities

HHN, RN Floaters: On-call for base office per office need/rotation.

HH&Hospice Nurse, RN Baylor: Responsible for working from 5pm Friday – 8am Monday; provide specialized hospice training to other staff, family members and informal caregivers; develop and implement the patient care plan in cooperation with other agency professionals; ensure complete documentation supporting hospice guidelines and COPs; provide regular supervision to LPNs and paraprofessionals; turn in all paperwork by 5:00pm Monday and communicate laptop daily Saturday and Sunday; make verbal/written report of new admissions and ER visits to primary nurse Sunday night or Monday morning between 8am-9am; collaborate with supervisor to meet assigned productivity standard; complete scheduled visits after 5pm on Fridays as warranted; work at least one weekend day.

HH&Hospice Nurse, RN Evening: Collaborate with supervisor to meet productivity of 100% for work hours 12pm-11pm Monday-Thursday; covers on-call for clinical office Monday-Thursday; visits done after 11pm will be paid at over quota rate and not considered part of quota.

HHN, RN Admissions: Provide home health admissions or tuck-ins for hospice as needed; develop and implement the patient care plan in EMR; ensure complete documentation supporting HH/hospice guidelines and COPs; turn in all paperwork to designated person in the office next day as needed and communicate iPad daily; make verbal/written report of new admissions and ER visits to primary nurse as needed; collaborate with supervisor to meet assigned productivity standard; complete scheduled visits as needed per productivity; on-call per needs of the office.

Requirements

  • Registered Nurse currently licensed to practice nursing in the state of North Carolina
  • Minimum of one-year experience as a professional nurse in an acute care setting
  • Maintain current CPR certification (BCLS)
  • Basic computer skills/knowledge and experience in electronic medical records
  • Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
  • Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized plan of care
  • Possess valid Driver's License and reliable transportation
