HACE Access and Impact Partner
Position Summary
The HACE Health Access & Impact Partner operationalizes City of Hope’s Health Access & Impact portfolio by translating systemwide health access and Social Determinants of Health (SDOH) strategy into implementation-ready workflows, governance materials, and measurable outcomes across clinical care delivery, resource navigation, community and internal partnerships, Epic-enabled execution, and accreditation requirements. The role is responsible for building and maintaining detailed project plans that define milestones, dependencies, risks, timelines, cross-functional owners, and deliverables across the Health Access & Impact portfolio.
Qualifications
- Bachelor’s degree in health administration, public health, nursing, business, analytics, or a related field preferred; equivalent combination of education, training, and progressively responsible professional experience may be considered in lieu of a degree.
- Five (5) or more years of experience in healthcare operations, program or project management, or clinical transformation initiatives, including cross-functional leadership.
Success Factors
- Portfolio Implementation & Project Management: Builds and maintains detailed project plans that define milestones, dependencies, risks, timelines, cross-functional owners, and deliverables across the Health Access & Impact portfolio.
- Data, Reporting & Continuous Improvement: Collects, validates, and organizes program data to support dashboards, reports, leadership updates, and continuous improvement across health access and impact initiatives.
- Governance & Stakeholder Alignment: Prepares status updates, recommendations, and decision-ready materials for HACE leadership and cross-functional stakeholders to support prioritization, sequencing, implementation, and scale decisions.
