Habilitation Coordinator

Position Summary

Burke is seeking a dedicated and compassionate Habilitation Coordinator to support individuals with intellectual and/or developmental disabilities (IDD) residing in nursing facilities. This position plays a vital role in coordinating services, facilitating person-centered planning, and helping individuals explore opportunities for greater independence and community integration.

Essential Duties & Responsibilities

• Conducting initial interdisciplinary team (IDT) meetings
• Convening and facilitating service planning team meetings
• Facilitating the development and ongoing revision of Habilitation Service Plans through a person-centered discovery process
• Cooking services and supports between the individual's Habilitation Service Plan and nursing facility plan of care
• Completing PASRR evaluations and related documentation
• Collaborating with nursing facility staff, service providers, individuals, and families
• Educating individuals, legally authorized representatives (LARs), and families about available community living options
• Aiding individuals transitioning from nursing facilities to community settings
• Developing Community Living Discharge Plans
• Maintaining accurate and timely documentation in accordance with program requirements
• Traveling throughout the service area to provide services and support

Education Requirements

• Required: Bachelor's or advanced degree from an accredited college or university with a major in a social, behavioral, or human services field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice
• Required: One (1) year of experience working or volunteering with individuals with intellectual and developmental disabilities (IDD), or Personal experience as a friend or family member of an individual with IDD

Experience Requirements

• Required: Valid Texas Driver's License

Licenses & Certifications

• Required: Required: Satori Alternatives to Managing Aggression (SAMA)
• Required: CPR/First Aid
• Required: Defensive Driving
• All other required training modules

Additional Requirements

• Reliable transportation
• Ability to travel throughout the service area
• Driving record acceptable to Burke's insurance carrier

Physical Requirements

• Candidates Must Be Able To Perform The Following:
• Evaluate and monitor consumer needs using visual and auditory skills
• Bend, reach, and move throughout various work environments
• Lift and carry up to 25 pounds, including charts and equipment
• Utilize fine motor skills for legible writing and computer documentation
• Aid consumers as needed during the course of service delivery

Schedule

Monday - Friday
8:00 a.m. - 5:00 p.m.
After-hours work may be required to meet consumer and program needs

Compensation

$42,120 - $43,555 annually
Salary Dependent Upon Qualifications And Experience
