Gynecology and Hematology Coordination Nurse

Responsibilities

  • Provide nursing support during onsite IHI Women's Specialty Clinic appointments, including patient intake, vital signs, medication reconciliation, procedure assistance, and patient education.
  • Support many of the pre-and post-visit responsibilities for weekly telehealth visits.
  • Carefully coordinate any pre-visit requirements, including obtaining consents, providing patient education, and facilitating pre-medication needs.
  • Triage incoming patient calls with a focus on women's health services and provide appropriate follow-up and after-hours care support.
  • Assess patient needs and communication nursing findings, recommendations, and care concerns promptly to providers, care team members, patients, and families in accordance with established triage protocols and clinical communication standards.
  • Serve as patient advocate by supporting patients and families, addressing barriers to care, and monitoring healthcare outcomes.
  • Collaborate effectively within a multidisciplinary care team and actively participate in the organization's patient-centered clinical care model.
  • Under the direction of a physician, educate patients on medications, treatment plan, and when to seek emergent care, while monitoring and reporting patient responses to treatment.
  • Communicate laboratory, diagnostic, and imaging results to patients and families in a clear, compassionate, and timely manner.
  • Manage medication prescriptions and refills under provider direction, including factor replacement therapies as applicable.
  • Utilize the electronic health record (EHR) to accurately review, document, and manage patient information while maintaining complete and compliant medical records.
  • Support ongoing initiatives, goals, and growth of the IHI Women's Specialty Clinic program through active participation in program development and quality improvement efforts.

Requirements

  • Graduation from an approved nursing program.
  • Valid RN nursing license, able to practice in the state of Indiana.
  • Minimum of 2 years of related nursing experience; preferably in collaborative chronic disease patient care, hematology, or gynecology.
  • Current BLS certification.
