Guided Access Primary Care

About the role

We are seeking innovative physicians to help establish Guided Access Primary Care Clinics within each of our Northeast Ohio submarkets (east, west, south of Cleveland) to support optimized care for our highest risk patients in value-based contracts.

Responsibilities

• Identify and manage high-risk patients through partnership with Value Based Operations team.
• Build a team-based practice to provide high touch primary care for a smaller panel of high-risk patients.
• Implement enhanced triage systems and integrated team approach, including embedded and centralized resources.
• Collaborate with other enterprise partners such as Medical Care at Home, Value Based Operations, etc.
• Utilize a fully integrated Epic electronic health record for care delivery.
• Focus on quality improvement and innovation in care delivery.
• Engage in teaching and leadership opportunities.

Requirements

• Board certified in Internal Medicine or Family Medicine.
• Experience in value-based care models.
• Strong interest in building and leading a team-based practice.
• Ability to work in a fast-paced, collaborative environment.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Current license to practice medicine in the state(s) of practice.
• Passing score on the COMLEX Level II-XP exam or equivalent.

Pay

Minimum salary: $240,500
Maximum salary: $359,750
