D&S Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. You'll join a mission-driven team and create relationships that motivate us all every day.
Responsibilities
- Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling, and oversight.
- Supervise a team of Caregivers supporting individuals we serve in the program.
- Provide support to individuals receiving services including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care.
- Duties are split between providing direct support, professional or program activities, and supervision.
- Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
Requirements
- Must have a full valid driver’s license held for at least 12 months post intermediate/permit and reliable vehicle with insurance and registration used to transport clients.
- Minimum 1 year of professional caregiving experience required.
- Must be able to provide full caregiver support, including meal prep, medication assistance, housekeeping, personal care, transfers, and total care using lifting equipment such as gait belts and Hoyer lifts.
Schedules
- Available: Full-Time.
- Must be available for on-call shifts, including nights, weekends, and 12–16 hour shifts.
- Shifts are based on client needs and may include: 16-hour overnight shifts (4:00 PM – 8:00 AM), Weekend shifts (Saturday and/or Sunday), Varying hours depending on staffing needs.
Qualifications
- High School diploma or equivalent.
- One year related work experience.
- Must be 18 years or older.
- Current driver's license, car registration, and auto insurance.
- Other licensure or certification where required by regulatory authority.
- Excellent communication skills with an ability to establish rapport with team members and those we serve.
- Strong organizational abilities to ensure staffing and schedules are maintained.
