Responsibilities

• Provide nursing care under the supervision of a Registered Nurse, including medication administration and treatments
• Absorb and implement resident care plans
• Maintain accurate documentation of resident care, treatments, and observations
• Monitor resident conditions and report changes promptly to the supervising nurse
• Assist with personal care tasks such as hygiene, mobility, and feeding support
• Maintain a safe and resident-centered environment
• Collaborate with nursing staff and interdisciplinary team members to support quality resident care
• Participate in team meetings and ongoing education opportunities
• Perform additional duties as assigned

Requirements

• High School Diploma or equivalent preferred
• Graduate Practical Nurse eligible for licensure in New York State
• Strong written and verbal communication skills
• Compassionate, resident-focused approach to care
• Ability to work effectively with residents, families, physicians, and team members
• Strong organizational and documentation skills
• Ability to perform frequent physical activity and assist with resident care needs
