About the role

Gilead Community Services is seeking an attentive and recovery-oriented Clinical Case Manager to join our multi-disciplinary team in our supervised apartment program for women. The program serves women with major mental illness, co-occurring substance use disorders and histories of significant trauma.

Responsibilities

  • Provide clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education
  • Work with the treatment team to manage clients' mental health and assess for risks, implementing risk management plans as necessary
  • Provide recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports
  • Provide support, skills training and education in areas including daily living skills, nutrition, financial management and symptom management
  • Monitor positive outcome measures and ensure program compliance through accurate and timely data reporting
  • Complete assessments, treatment plans, and other documentation electronically and on paper in a timely manner
  • Act with compassion, integrity, and ethics

Requirements

  • Bachelor’s degree in a human service field; Master’s degree preferred
  • One year of clinical experience
  • Maintain and provide proof of valid driver’s license and automobile insurance in good standing
  • Reliable use of personal vehicle as needed
  • Experience with trauma treatment, and co-occurring psychiatric and substance abuse disorders preferred
