Key Responsibilities
- Conduct comprehensive in-home assessments
- Develop and oversee individualized care plans
- Career and monitor medical care, home care, and community services
- Accompany clients to medical appointments and advocate when needed
- Provide ongoing support, guidance, and education to families
- Assist with care transitions (hospital, rehab, assisted living, memory care)
- Maintain consistent communication with families and care teams
- Serve as a trusted liaison between families, providers, and caregivers
Qualifications
- LSW, or LCSW license required (New Jersey)
- Experience working with older adults, dementia, and complex medical needs
- Strong knowledge of healthcare systems, long-term care options, and community resources
- Excellent communication and relationship-building skills
- Ability to work independently with a high level of professionalism and discretion
- Reliable transportation required
- Certification as a Geriatric Care Manager (CCM, CMC, or equivalent) is highly desirable
