Qualifications

• Associate Degree or higher
• Social Service Experience Preferred
• Certified Dementia Practitioner
• Management Experience in Long Term Care Preferred

Responsibilities

• Complete MDS and other needed assessments
• Manage staff and day to day operations on memory care unit
• Run care plan meetings for residents on memory care unit
• Ensure daily programming is taking place
• Assist with daily programming when needed
• Work with other department leaders as needed to ensure smooth operation of the memory care unit
• Educate staff on dementia unit related to dementia and programming
• As well as other duties that arise
