Each client is assigned carefully screened primary and back-up caregivers based on their interests and needs.

About the role

Primecare Home Care is seeking a dedicated and detail-oriented GAPP Care Coordinator to support and coordinate care for Pediatric clients eligible for the Georgia Pediatric Program (GAPP).

Responsibilities

  • Identify Pediatric clients eligible for the GAPP program and assist them in accessing program benefits.
  • Coordinate with clients, parents, and physicians to ensure all required documents for state enrollment are completed accurately and in a timely manner.
  • Act as the primary point of contact for clients and families, addressing questions or concerns regarding the GAPP program.
  • Ensure all required documentation, including state documents and program renewals, are completed and submitted according to state guidelines and deadlines.
  • Maintain accurate records and documentation of client interactions and program enrollment status.
  • Collaborate with internal teams to facilitate seamless communication and coordination of care for GAPP clients.
  • Stay up-to-date on Medicaid state-funded programs and regulations, as well as home care services, to effectively support GAPP clients.

Requirements

  • High School Diploma or equivalent.
  • Strong organizational skills with the ability to manage multiple tasks and deadlines effectively.
  • Excellent communication and interpersonal skills, with the ability to interact professionally with clients, families, and healthcare providers.
  • Detail-oriented approach to documentation and record-keeping.
  • Ability to work independently as well as part of a team, demonstrating flexibility and adaptability in a fast-paced environment.

Qualifications

  • Bachelor's degree in healthcare administration, social work, or a related field (preferred).
  • Previous experience working with Medicaid state-funded programs and/or home care services (preferred).
  • Knowledge of Georgia Pediatric Program (GAPP) guidelines and procedures (preferred).
  • Proficiency in understanding the unique needs and challenges faced by children with physical, functional, and mental disabilities.
  • Strong English written and verbal communication skills, including accurate grammar and business correspondence knowledge.
  • Ability to read, analyze, and interpret regulations and other documents.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

Pay

$50,000

Physical Demands

  • Frequently required to sit, stand, walk, use hands to finger, handle, or feel objects, tools, or controls, and reach with hands and arms.
  • Requires manual and finger dexterity and eye-hand coordination.
  • Frequent use of computers and phones.
  • Ability to lift/carry up to 30 pounds using appropriate body mechanics.
  • Requires corrected vision, including close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Requires hearing within normal range and effective verbal and written communication skills.

Work Environment

  • Primarily indoors in a typical, temperature-controlled office environment with moderate noise levels.
  • May experience distractions such as phone calls, work interruptions, and communication from co-workers or participants.
  • May require working under stressful conditions to meet scheduled timelines.
