Responsibilities

• Evaluate and treat patients by following an individualized plan of care after evaluation.
• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.

Requirements

• Experience administering standardized speech/language assessments.
• Conduct swallowing evaluations.
• Develop treatment plans.
• Monitor patients' progress.

Skills

• Strong written, oral, and interpersonal skills.
• Ability to work well with others and communicate effectively with patients, families, and treatment team members when gathering and exchanging patient condition information.
