Responsibilities

• Evaluate and treat patients by following an individualized plan of care following an evaluation.
• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.

Requirements

Strong written, oral and interpersonal skills; work well with others and communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.

Qualifications

• Experience administering standardized speech/language assessments
• Conduct swallowing evaluations
• Develop treatment plans
• Monitor patients' progress
