Full Time Service Coordinator
About the role
The Service Coordinator at Presbyterian Villages of Michigan will assume responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. They will assess and identify resident and family needs and seek out outside resources to meet those needs. This position involves networking with community organizations and seeking new services to offer residents.
Responsibilities
- Assists in assessing and identifying resident and family needs and outside resources to meet those needs.
- Initiates and completes psychosocial assessments, interviewing patients, families and others to evaluate current condition and identify current or potential adjustments.
- Provides general case management (and referral services) and maintains complete and updated resident files.
- Maintains supportive services where necessary.
- Captures and manages services for optimal living (i.e., aging in place and wellness philosophies).
- Completes Care Plans/Quarterly Monitoring with residents needing referrals and follows up on all services at and after implementation.
- Sends Semi-Annual Performance Reports to HUD to maintain funding for the Service Coordination program at the property.
- Engages all residents to identify areas of need, and makes necessary referrals.
- Organizes and promotes resident involvement in on and off-site programs.
- Aids in the coordination of home care services to enable independent living and aging in place.
- Develops a Resource Directory that includes a list of service providers.
- Continuously educates residents (and staff) on a variety of matters including service availability, application procedures, resident rights, health/safety/fitness, issues relating to aging, etc.
- Coordinates counseling, occasional visits from a nurse, preventative health screenings/wellness, and legal advocacy.
Requirements
- Minimum associate's degree or equivalent in social work, psychology, or gerontology preferred.
- Two or more years of experience in social service delivery with the elderly and/or family population is a plus.
- Demonstrated working knowledge of supportive services and other resources in the area served by the project.
- Demonstrated ability to advocate, organize, problem-solve, and provide results for the residents served.
Skills/Abilities
- Good communication, comprehension, and interpersonal skills.
- Working knowledge of Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, RealPage, OneSite, or similar related systems, etc.).
