About the Role

Deliver and coordinate in-home nursing care, ensuring compliance with laws and policies. Conduct clinical assessments, develop individualized care plans, and collaborate with healthcare professionals to ensure continuous patient care.

Responsibilities

• Provide high-quality clinical services within scope of practice and infection control standards
• Coordinate care with the patient’s care team from admission to discharge
• Complete clinical assessments according to program and payer requirements
• Verify patient eligibility and medical necessity for services
• Develop and update care plans in collaboration with community providers
• Adjust care plans as needed with physician approval
• Function effectively in diverse home environments, regardless of client background

Requirements

• Current, unrestricted RN licensure in the practice state
• Valid driver’s license, vehicle insurance, and reliable transportation

Preferred Skills

• CPR certification
• 1+ years RN experience
• Strong communication and organizational skills
• Ability to work independently

Pay

Pay ranges from $39.45 to $59.15 per hour, based on experience and location.
