Responsibilities
- Performing patient assessments and administering medications
- Managing chronic conditions and supporting primary care providers
- Coordinating services across medical, dental, behavioral health, and wellness programs
- Providing patient education and triage
- Documenting patient information and collaborating with an interdisciplinary team
Requirements
- Valid RN licensure
- Experience in a community health or similar environment preferred
- Strong communication and teamwork skills
