About the role

Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our dynamic team. You will be part of a growing team providing the highest quality care for patients in a Skilled Nursing Facility (SNF) setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Provide excellent care and customer service to patients and family members
• Collaborate with facility staff to ensure excellent care for patients
• Evaluate new patients, develop plans of care, and set measurable, functional goals
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress, revise plans of care, and reset goals
• Complete timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes

Pay

$78,000–$90,000 per year, plus a sign-on bonus.
