Enhance Rehabilitation is currently seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Provide excellent care for our patients
• Provide excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluate new patients, develop plans of care and set measurable, functional goals
• Treat patients, answer their concerns and educate them on their injury, progress and probable outcomes
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress, revise plans of care and reset goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges, and daily progress notes
