About the Role

We are seeking a Physical Therapist to deliver high-quality in-house therapy services to adults with a range of diagnoses and disabilities, with a special focus on supporting aging in place. In this role, you will directly influence residents' quality of life by providing person-centered interventions that promote mobility, independence, and overall well-being. Responsibilities

  • Conducts screenings of new community residents and those referred by nursing to determine the need for intervention
  • Evaluates residents to obtain data necessary for treatment, planning, and implementation
  • li>Interprets and communicates evaluation findings to residents, family, and other team members
  • Records treatment times and documentation by following federal and state laws, rules, and regulations
  • Demonstrates capacity to understand insurance reimbursement and billing while coordinating care
  • Provides treatment in accordance with established standards of practice, department procedures, and productivity standards while adapting treatment for individual needs
  • Monitors resident response to intervention, modifying treatment as indicated to attain goals in accordance with physician orders
  • Recommends and educates residents, family, and community staff in the use of adaptive equipment and assistive devices
  • Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs, including excellent communication with community staff
  • Maintains safety standards, including compliance with infection control, universal precautions, and OSHA standards for healthcare professionals

Qualifications

  • Graduate of an accredited therapy program - Physical Therapist (PT)
  • Current state licensure
  • Excellent verbal, written, and interpersonal communication skills
  • Results-oriented with strong critical thinking and problem-solving skills
