We are seeking a Full-Time Physical Therapist (PT) to join our rehabilitation team in Charlevoix, MI. Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide skilled PT interventions
• Improve strength, balance, mobility, and functional independence
• Complete accurate clinical documentation
• Communicate with nursing and interdisciplinary team members
• Participate in care planning and discharge planning
• Educate patients and caregivers regarding treatment and safety

Requirements

• Current Michigan Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Strong clinical and communication skills
• SNF experience preferred but not required
