Full Time Physical Therapist (PT)

About the role

Join our dynamic team as a Physical Therapist, providing the highest quality care for patients in a Skilled Nursing Facility (SNF) setting. You will evaluate and treat patients following an individualized plan of care, ensuring excellent care and customer service to patients and their family members.

Responsibilities

• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress, revise plans of care, and reset goals as needed.
• Complete timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes.
