Responsibilities

• Develop and implement individualized treatment plans for patients
• Assess patient progress and modify treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Provide education and training to patients and families on self-management techniques
• Participate in staff meetings and continuing education opportunities

Requirements

• Masters degree in Physical Therapy from an accredited program
• Current license to practice as a Physical Therapist in the state of Indiana
• At least one year of clinical experience in a healthcare setting
• Excellent communication and interpersonal skills

Qualifications

• Strong clinical skills and knowledge of physical therapy principles
• Ability to work independently and as part of a team
• Passion for working with older adults and promoting health and wellness
