Full time Oasis Nurse

Responsibilities

• Complete Home Health Start of Care (SOC) admissions in patients' homes.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Accurately complete OASIS documentation and all required clinical records.
• Provide clinical support in the office when not conducting admissions.
• Ensure compliance with Medicare regulations, agency policies, and home health standards.
• Collaborate with the interdisciplinary team to provide high-quality patient care.
• Deliver compassionate, patient-centered care while promoting safety and independence.

Qualifications

• Active Massachusetts Registered Nurse (RN) license.
• Valid driver's license and reliable transportation.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently and effectively manage time in a fast-paced environment.
• Excellent organizational and problem-solving abilities.

Preferred Qualifications

• Home health experience preferred.
• OASIS documentation experience preferred.
• Spanish-speaking is a plus but not required.

Pay

$1,000 sign-on bonus after 90 days of employment when you successfully complete your introductory period.
