About the role

We are seeking a Home & Community Care Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage members. This role offers flexible scheduling between 8am-8pm, with no on-call, weekends, or holidays, promoting work-life balance.

Responsibilities

• Perform comprehensive assessments, including history review, physical exams, and screenings.
• Identify diagnoses, document findings, and communicate with primary care providers.
• Recognize urgent situations and provide patient education on health management and social needs.
• Address social determinants of health and facilitate referrals.

Requirements

• Active licensure and certification (ANCC/AANP for NPs, NCCPA for PAs).
• Ability to obtain or practice independently if permitted.
• Reliable transportation and ability to navigate home environments.

Qualifications

Preferred qualifications include:

• 1+ years of clinical experience.
• Proficiency with EMRs.
• Strong communication skills.

Pay

Compensation range from $109,500 to $164,000, including base pay, bonuses, and benefits.
