About the role

We are seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare and plan members. The role offers flexible scheduling within 8am–8pm, with no on-call, weekends, or holidays, promoting work-life balance.

Responsibilities

• Perform comprehensive assessments, including medical history, physical exams, and screenings
• Document diagnoses and communicate with primary care providers
• Recognize urgent situations and provide patient education on disease management and social factors
• Refer patients to appropriate services as needed

Requirements

• Active licensure and certification (ANCC, AANP, or NCCPA)
• Unrestricted driver’s license and reliable transportation
• Ability to navigate home environments and perform physical assessments

Qualifications

• Preferred: 1+ years clinical experience
• Preferred: Proficiency with electronic records
• Preferred: Strong communication skills with Medicare populations
