Full-Time Nurse Practitioner

Join a compassionate healthcare team as a Full-Time Nurse Practitioner providing primary care to elderly participants in a community-based program. Responsibilities Include Performing physical assessments for new enrollees and semi-annual re-evaluations.Developing and implementing individualized care plans in collaboration with an interdisciplinary team.Evaluating participant complaints and delivering appropriate treatment.Providing education and support to participants and caregivers.Overseeing resident care in skilled nursing facilities and coordinating with primary care physicians and medical directors.Participating in discharge planning and managing emergent needs. Qualifications Licensed RN and certified Nurse Practitioner, with at least one year experience working with frail and elderly populations.Graduate of an accredited nursing school.Preferred: Geriatric Nurse Practitioner certification and two years of geriatric experience. Skills & Attributes Strong assessment, planning, and care coordination skills.Ability to work effectively within a team and communicate clearly.Willingness to travel locally to participant homes. Additional duties: Oversight of residents in skilled nursing facilities, participation in interdisciplinary team activities, and contributing to discharge planning.
