Highlights of the Memory Care Coordinator Position
- Oversight of the overall implementation, delivery and coordination of the Connections Memory Care neighborhood resident services
- Engagement and supports the MCD/HWD in non-clinical health services
- Coordinates engagement aspects of life for Memory Care residents including participating in the development of personalized plan or program for successful aging in community
- Ensures residents satisfaction and achievement of their realistic goals
- Motivates residents to function at highest level of independence
