Responsibilities

  • Coordinate and complete MDS assessments and care plans
  • Ensure accurate and timely submission of MDS data to Medicare and Medicaid
  • Collaborate with interdisciplinary team members to develop and implement care plans
  • Monitor residents' progress and adjust care plans as necessary
  • Provide education and support to staff and families regarding care plans and resident needs
  • Assist with RAC-CT certification process
  • Generate and analyze casper reports to identify areas for improvement
  • Participate in quality improvement initiatives

Requirements

  • Current Georgia RN or LPN License
  • Two years of MDS experience in SNF setting preferred
  • RAC-CT Certification preferred

Qualifications

  • Knowledge of MDS/CAAs/Care plan completion for all disciplines
  • Knowledge of Georgia Case mix and Medicare PDPM
  • Knowledge of Medicare Cert and Re-cert completion
  • Detail-orientated and excellent time management skills
  • Critical thinker/Problem solver
  • Foster teamwork with all departments
  • Strong organization skills

Pay

Competitive salary commensurate with experience

Schedule

Dayshift, Monday through Friday, 40 hours per week
