Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing treatments and procedures as required
• Maintain accurate and up-to-date patient documentation and records
• Coordinate care and communicate effectively with patients, families, and interdisciplinary teams
• Ensure compliance with all safety, infection control, and regulatory standards
• Participate in patient education and advocate for patient needs

Requirements

• Current and valid Registered Nurse license for the state of Michigan
• Minimum of 1 year of nursing experience, preferably in home health care
• Ability to work independently and manage a diverse patient caseload
• Strong communication and interpersonal skills
• Reliable transportation and valid driver's license
• CPR certification
• Ability to pass drug screening and background check
