Role Overview: A registered nurse provides individualized, in-home nursing care to promote recovery, safety, and independence for patients. Key Responsibilities Serve as the primary clinical partner, delivering personalized care and conducting comprehensive home assessments.Perform skilled interventions such as wound care, infusions, medication management, and chronic disease care.Educate and empower patients and caregivers on symptom management, medication use, and daily care routines.Monitor patient progress, identify early changes, and communicate with physicians and care teams to adjust care plans.Coordinate care across providers to ensure continuity and high-quality outcomes.Complete accurate documentation to support compliance, quality, and measurable results. Qualifications & Skills Active RN license in the practice state.Minimum of one year of clinical nursing experience.Patient-focused, organized, and motivated to deliver high-quality care.Valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
