Full-Time Home Health Physical Therapist

Position Overview Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover in their own homes. Responsibilities Include Performing initial and ongoing assessments of mobility, strength, and function.Administering and interpreting tests to set functional goals and outcomes.Providing written evaluations and care plans outlining needs, goals, and progress.Delivering physical therapy services per physician orders and documenting appropriately.Maintaining and updating the plan of care, modifying goals, and planning discharges.Collaborating with healthcare team members, educating patients and caregivers, and coordinating services.Supervising personnel such as PTAs and HHAs when applicable.Completing OASIS assessments in accordance with regulations.Ensuring compliance with policies, procedures, and legal standards. Qualifications And Skills Degree in Physical Therapy from an accredited program.Current, unrestricted state license.Proficiency in assessments, documentation, and compliance.Valid CPR certification, driver’s license, and automobile insurance.Experience in home health therapy and OASIS documentation preferred.
