Full-Time Home Health Physical Therapist

Position Overview Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover in their own homes. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, and administering tests to determine goals and outcomes. Develop and document personalized plans of care, deliver prescribed physical therapy services, and modify treatment plans as needed. Responsible for discharge planning and educating patients and caregivers on home exercise programs. Coordinate with healthcare teams, document accurately to ensure compliance, and communicate effectively with staff, patients, and families. Supervise personnel such as PTA and HHA where applicable, and complete assessments like OASIS following regulations. Adhere to all policies, standards, and legal requirements.
