About the role
The LPN is responsible for providing quality patient-centered, evidenced-based care to our residents. Duties include initial data collection and participating in the implementation of person-driven care plans.
Responsibilities
- Utilize principles of Relationship Based Care
- Perform duties as assigned
- Initial data collection on all residents
- Participate in implementing person-driven care plans
Requirements
- Current licensure as a Practical Nurse in good standing
- Must be able to read, write, and communicate in English
- Clinical experience preferred
Qualifications
- Dynamic and compassionate
- Experience in senior care preferred
Skills
- Strong communication skills
- Ability to work independently and as part of a team
- Knowledge of evidence-based practices

Pay
- Starting salary range: $27.55 - $31.19 per hour
