About the role

Utilize the nursing process to plan, implement, and evaluate patient care for adult clients in their homes. Develop and revise care plans based on patient needs and physician directives. Administer medications and treatments, monitor effectiveness, and report changes to healthcare providers. Provide therapeutic, preventive, and rehabilitative nursing actions aligned with patient diagnoses and goals. Identify educational, care coordination, and discharge planning needs through review of care protocols. Respond to patient condition changes and intervene as necessary, ensuring appropriate documentation.

Responsibilities

• Plan, implement, and evaluate patient care for adult clients in their homes

Qualifications

• Current RN license for the practicing state
• Valid health certificate, PPD or chest X-ray, and BLS certification
• Preferably one year of RN experience
