About the Role

Provide each assigned resident with routine daily nursing care and services in accordance with the resident's assessment and care plan, as directed by supervisors.

Responsibilities

  • Use wristband or photo card file to identify residents before administering treatments, serving meals, etc.
  • Use only authorized abbreviations when recording information.
  • Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse.
  • Record all entries on flow sheets, notes, and charts in an informative, descriptive manner.
  • Report all accidents and incidents observed during the shift.
  • Agree not to disclose resident's protected health information and report suspected violations.
  • Report unauthorized attempts to access the facility's information system.
  • Perform only nursing care procedures for which you have been trained.
  • Prepare resident's room for their arrival (bed made, name tags, admission kit available).
  • Greet residents, escort them to their room, and introduce them to roommates and personnel.
  • Inventory and mark resident's personal possessions as instructed.
  • Assist residents with packing personal possessions for transfers or discharges.
  • Transport residents to new rooms or receiving areas and assist with loading/unloading to/from vehicles.
  • Maintain an atmosphere of warmth, personal interest, and a calm environment.
  • Meet with nursing personnel to identify and correct problem areas or improve services.
  • Report all resident complaints and grievances.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds, measure and record temperatures, pulses, respirations (TPRs), and weights as instructed.
  • Check residents unable to call for help frequently and answer resident calls promptly.
  • Assist residents with food arrangements and record food/fluid intake.
  • Report changes in resident's eating habits to the Nurse Supervisor/Charge Nurse.
  • Keep residents' water pitchers clean, filled, and within easy reach.
  • Perform after-meal care (remove trays, clean resident's hands, face, clothing).
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules to complete assigned tasks.
  • Cooperate with inter-departmental and facility personnel to maintain nursing services.
  • Notify the facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bathing, dressing, combing hair).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests (lab work, x-ray, therapy, dental).
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Prepare residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met.
  • Observe and report pressure areas, skin breakdowns, and injuries of unknown source.
  • Observe disoriented and comatose residents and record/report data as instructed.
  • Perform special treatments as instructed.
  • Report any resident leaving or missing from the facility immediately.
  • Use only equipment you have been trained to use and report defective equipment.
  • Maintain confidentiality of resident care information and report unauthorized disclosures.
  • Report allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment and report such requests to your supervisor.

Requirements

  • Minimum 10th grade education.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions.
  • Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions.
  • Ability to solve practical problems with limited standardization.
  • Ability to interpret instructions in written, oral, diagram, or schedule form.
  • Licensed Certified Nursing Assistant in accordance with state laws.
  • CPR certification preferred.

Physical Demands

  • Occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office equipment (copier, scanner, fax, telephone, calculator).
  • May assist in evacuating residents during emergencies.

Work Environment

  • Noise level is usually low to moderate.
