About the role
This position involves providing home care nursing services within a community health setting, primarily in patient homes and office environments.
Responsibilities
- Deliver direct patient care and coordinate services in home settings.
- Supervise and collaborate with a multidisciplinary team including RN Case Managers, Visit RNs, LPNs, PTs, OTs, RDs, SLPs, MSWs, and Clinical Schedulers.
- Maintain compliance with HIPAA and ensure confidentiality of patient information (Level 4 access).
Qualifications
- BSN and at least two years of home care experience or equivalent education/experience.
- Current license and registration to practice as a registered nurse in New York State.
- Ability to work in team settings and adapt to both office and field environments.
