Job Title: Case ManagerReports to: Director of Housing ProgramsLocation: Providence, RI (On-site at Haus of Codec and in the field)Status: Full-Time (40 hours/week)Hours: 10 AM - 6 PM, Monday-Friday, with some flexibilityCompensation: $28/hourAbout Haus of CodecHaus of Codec is a mission-driven organization working to end transition-aged youth homelessness in Providence through housing, arts, and workforce development. We specialize in supporting LGBTQ+ and BIPOC youth, providing safe housing and creative outlets.Position SummaryFull-Time Case Manager with a certified Community Health Worker certification (CCHW) is a key support role focused on improving the well-being of a caseload of transition-aged youth clients in our Emergency Shelter and Joint Transitional Housing/Rapid ReHousing programs, some of whom live in Haus of Codec’s three-story residence in Providence, and the remainder of whom live in scattered site apartments, primarily in Providence, but also in other towns in Rhode Island. This position combines traditional case management with a public health lens, helping clients navigate healthcare, housing, and social services while also promoting safe and healthy living conditions.You’ll join one other Full-Time Case Manager with their own caseload, working closely with clients in our Joint TH/RRH program to achieve health and wellness goals, connect them to services, coach them in job searches and life skills, and build community through outreach and engagement. You’ll do this, in part, by spending 2-3 days per week at the residence where Emergency Shelter and Transitional Housing clients reside. Another major part of this role includes the ability to travel to Rapid ReHousing clients’ apartments, some of which are in the East Bay or near the Connecticut border, to conduct home visits. Finally, a successful Full-Time Case Manager would also be involved in doing direct outreach, becoming a familiar, trusted presence at community sites and events. Key ResponsibilitiesCase Management & Client SupportCase Management & client supportConduct intakes and develop individualized service plansAssist clients with housing navigation, job search, education, and public benefitsSupport access to primary care, behavioral health, and other community servicesDocument progress and maintain case files using internal systems and the Homelessness Management Information System (HMIS) / ClarityParticipate in regular check-ins and care coordination meetings internally and with providers at other organizationsAttend apartment viewings and provide logistical support for clients moving into new apartments, when neededPromote awareness of how the physical and social environment impacts healthHelp clients set and work toward personal health-related goalsConnect residents to resources through the Healthy Communities Office, Arts & Health initiatives, and Community Health Worker Association of RI (CHWARI)Engage clients with activities that build community and resilienceProvide timely support in crisis situationsAdvocate for clients with service providers, landlords, and public agenciesModel trauma-informed and harm reduction approachesAttend team meetings, supervision sessions, and professional development trainings QualificationsRequiredCHW Certification (CCHW) or enrollment in a CHW programFluency in English (verbal and written)Experience in case management, community outreach, healthcare, or social servicesPassion for youth empowerment, especially within LGBTQ+ and BIPOC communitiesStrong interpersonal and communication skillsComfort supporting individuals with mental health or substance use challengesCompetency using Google Workspace and general office toolsAbility to lift up to 50 lbs and work on-site/in the fieldExperience handling confidential informationAbility to work on-site at our residence or co-working space (monthly bus pass will be provided)Valid driver’s license and access to reliable transportation, with the ability to conduct home visitsPreferredExperience living in or working with people living in public housingBilingual in additional languages (especially Spanish, potentially Haitian Creole, Cape Verdean Creole)Self-starters and community-minded folks encouraged to applyKnowledge of harm reduction principles, Housing First, and practices of cultural humility Commitment to EquityHaus of Codec is an equal opportunity employer.
