About the role

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent will provide education and resources to support patient care progression, utilization of services, and safe patient transitions for patients, families, and healthcare teams.

Qualifications

• Minimum of one year of hospital‑based Case Management experience or five years in healthcare
• Graduate of an accredited nursing program
• Hold Basic Life Support (BLS) Healthcare Provider certification upon hire
