Full-Time Case Manager

Responsibilities

• Coordinate comprehensive patient care across emergency, surgical, mental health, and rehabilitation services
• Assess patient needs and develop individualized care plans
• Collaborate with an interdisciplinary team to facilitate safe transitions and positive outcomes
• Connect patients and families with community resources
• Address social determinants of health and ensure thorough documentation
