Duties & Responsibilities
- Facilitate and lead the process for engaging families, community members, and service providers in critical decisions related to safety and risk.
- Accurately track FTC information, reschedules and cancellations.
- Contribute knowledge about the needs, strengths and resources which may be present to ensure the child(ren)'s safety while supporting the family and giving them access to services in the community.
- Contact and consult with ACS, agency personnel and service providers as needed to schedule ACS related (Trial Discharge, Final Discharge, Parole, Placement Change) FTC’s.
- Lead conference participants in the group process and become involved in the discussion needed to ensure the best decision in regard to the safety and well-being of the child by facilitating a discussion of permanency through shared decision making to develop a plan with the family to ensure the safety, well-being and permanency for the child.
- Effectively manage conflict and strong emotions as they arise by utilizing conflict management skills while directing the group process and structure of the conference.
- Accurately record FTC information and decisions on designated forms (FTC Summary Report) and complete the Service Plan Review tab in Connections within 48 hours of each conference.
- Promote an inclusive, safe and supportive environment where all participants will be assured that they can and will be heard.
- Works with accountability department to ensure documentation of all FTC is completed on a timely basis.
- Debrief with case planning staff following each conference to ensure the goal of conference are met.
Education & Experience
- Bachelor’s degree in social work or a social service field with at least 2 years experience working in child welfare. At least 1 year of Family Team Conferencing experience preferred.
