About the role
The CCBHC Service Navigator helps individuals who are seeking services with MHR by reviewing intake screeners and making referrals to community resources and service programs as indicated by their screeners. The navigator is expected to have a general understanding of all programs offered by the CCBHC so that clients can be educated about the programs and connected effectively to care that best fits their recovery goals.
Responsibilities
- Meet with clients upon intake to review all intake screeners, address SDOH needs and link the client to an appropriate community resource to reduce SDOH factors.
- Maintain a working knowledge of all CCBHC services and refer individuals to programs within the CCBHC that can support immediate needs and reduce barriers to care.
- Conduct brief intervention with clients regarding problematic alcohol use when indicated by the AUDIT‑C screener.
- Discuss programs or groups offered at the agency or within the community to support the client’s recovery needs.
- Complete the Health Screening with the client, obtain basic health information (blood pressure, weight) and help navigate a referral to a primary care physician if needed.
- Assess a client’s desire to create an advance directive and link the client to the agency resource for completion.
