FSC Support Coordinator

About the role

Coordinate early-intervention services for infants and toddlers with developmental delays or disabilities and their families under Louisiana’s EarlySteps program.

Responsibilities

• Participate in the initial completion of a child’s Individualized Family Service Plan (IFSP) with the System Point of Entry (SPOE).
• Coordinate all services across agency lines and serve as the single point of contact to help parents obtain the services and assistance identified on the IFSP, including medical, advocacy, and other needed providers.
• Assist parents in gaining access to every service listed on the IFSP.
• Assemble and convene multidisciplinary eligibility teams for annual assessments or whenever new concerns arise.
• Ensure IFSP teams are established to determine appropriate service levels and that services are delivered on time.
• Verify that IFSP services continually meet the child’s and family’s evolving needs identified during assessments, monthly phone contacts, and quarterly face-to-face visits.
• Coordinate the provision of early-intervention services and any other required services (e.g., medical services beyond diagnostic/evaluation purposes).
• Oversee the transition of each child out of EarlySteps at age three, upon loss of eligibility, or when all outcomes have been achieved.
• Maintain required professional licenses/certifications and complete annual mandated training.

Requirements

• Bachelor’s degree in Social Service, Human Services, or a related field; or a Bachelor’s degree with at least 16 credit hours in Social or Human Services.
• Own reliable transportation, current driver’s license, and auto insurance for regular participant visits.
• Willing and able to travel during the day to outlying parishes as required.
• Ability to multitask and meet deadlines.
• Excellent written and verbal communication skills.
• Proficient in standard office software (Word, Excel) and computer literate.
• Team-player mindset and supportive of all team members.
• Successfully complete all state-mandated training and pass required certification testing.
