FREEDOM NAVIGATOR ROLE

About the role

The Freedom Navigator is an impacted professional who uses lived experience to support individuals transitioning from incarceration back into the community. Acting as a mentor, advocate, and resource connector, the Freedom Navigator helps Freedom Fellows access housing, employment, education, healthcare, recovery services and other essential support. Through guidance, accountability, and wraparound support, Freedom Navigators help fellows overcome barriers, meet supervision requirements, and build a foundation for long-term stability, independence, and successful community reintegration.

Responsibilities

• Freedom Fellow Engagement and Relationship Building
  • Meet Freedom Fellows where they are while promoting personal responsibility and goal achievements
  • Establish trusting, professional relationships with Freedom Fellows through consistent engagement and support
  • Provide ongoing encouragement, motivation, and accountability throughout the Freedom Fellow’s reentry journey
  • Maintain regular contact through in-person meetings, phone calls, virtual meetings, and community meetings
• Resource Connection & Coordination
  • Connect participants to community-based services and support such as housing providers, legal/court systems, employers and training programs, healthcare providers, and community organizations
  • Support Freedom Fellows in obtaining identification, benefits, transportation, and other resources essential to independence and stabilization
  • Advocate beside the Freedom Fellow to reduce barriers and increase access to opportunities
  • Coordinate referrals to housing providers, employers, educational programs, healthcare providers, treatment services, legal resources, and social service agencies
• Ongoing Support
  • Dedicate up to 15 hours per week engaging in team and one-on-one meetings and/or connecting with community partners
  • Identify barriers to stability and connect Fellows to appropriate resources and support
  • Work with Fellows and MFF reentry team to problem solve barriers Fellows face
  • Celebrate Fellows milestones and progress
• Documentation
  • Maintain case notes
  • Track Fellow progress toward reentry stability and independence
  • Document connections made for Fellows to services and support

Requirements

• Lived experience with incarceration
• Strong communication and relationship-building skills
• Ability to work effectively with diverse populations
• Ability to maintain professional boundaries and confidentiality
• Demonstrated commitment to supporting individuals impacted by the criminal justice system
• At least two years of demonstrated stability following release from incarceration
• High School or Equivalent
• 2 – 5 years of experience

Pay

$20.00 – $27.00 Per Hour
