Foster Care Case Manager
About the role
Massachusetts Mentor, a part of the Sevita family, provides Residential and Treatment Foster Care services to youth facing various out-of-home placements, transitions, or involvement in the juvenile justice system. Our personalized programming aims to help youth gain stability, comfort, and confidence to overcome challenges and thrive.
Responsibilities
- Provide case management and counseling services to an assigned individual caseload.
- Assess referred individuals and lead development, coordination, and evaluation of service plans in collaboration with treatment team, funding sources, and key stakeholders.
- Maintain current and complete records in accordance with corporate, contractual, and regulatory standards.
- Monitor and consult with mentors regarding the implementation of service plans, including pre-service introductions and continuing education.
- Make mentor and family home visits, and conduct mentor group meetings as required.
- Assist with emergency coverage and crisis intervention support, including participation in a rotational on-call system.
- Aid in legal, licensing, regulatory, and certification activities as needed.
- Participate in activities to enhance and build an active network of mentors.
Qualifications
- Bachelor’s degree in human services or related field.
- One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc., preferred.
- Current driver's license, car registration, and auto insurance.
- Licensure and training as required by state, such as CPR, First Aid, Behavioral Intervention Techniques, etc.
- A reliable, responsible attitude and a compassionate approach.

Pay
Foster Care Case Manager: $50,000
Schedule
Full-time position with flexible scheduling to accommodate the needs of the youth and families served.
