Responsibilities

- Assist in providing services to individuals with disabilities. - Conduct assessments and develop individualized service plans. - Facilitate access to community resources and support systems. - Maintain accurate records and documentation. - Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

- Bachelor's degree in a related field (social work, psychology, etc.) or equivalent experience. - Valid driver’s license and reliable transportation. - Excellent communication and interpersonal skills. - Ability to work independently and as part of a team. - Commitment to diversity, equity, and inclusion.

Qualifications

- Experience working with individuals with disabilities. - Knowledge of community resources and support systems. - Strong organizational and time management skills. - Proficiency in Microsoft Office Suite.

Pay

Salary range based on experience and qualifications.
