We are seeking a dedicated and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our team at Springfield Center for Living and Rehabilitation in Claremont, New Hampshire. As an LPN on our evening shift, you will play a vital role in providing high-quality patient care and support to our residents.

Responsibilities

• Provide direct patient care to assigned residents, including vital sign monitoring, wound care, and medication administration
• Collaborate with other healthcare professionals to develop and implement individualized care plans
• Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
• Monitor and report any changes in patient condition or behavior to the Charge Nurse or Physician
• Maintain accurate and detailed records of patient care and treatments
• Participate in quality improvement initiatives to ensure optimal patient outcomes

Requirements

• Current licensure as a Licensed Practical Nurse (LPN) in the state of New Hampshire
• High school diploma or equivalent required
• Previous experience in a long-term care or rehabilitation setting preferred
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to patient care
